The Community Hospice Foundation works through ThedaCare At Home™ to help people conclude their life with comfort and dignity by offering pain and symptom control in a loving environment of their choice.

We are a non-profit organization whose mission is to promote and financially support quality hospice and home care services and programs in the Fox Valley and ThedaCare At Home™.

Our work over the past 22 years has included:

Our quilt (click above to enlarge), created by hospice residents, that is draped over a deceased member to honor their passing.

• Providing financial assistance to families unable to afford hospice and home care.
• Supporting educational opportunities for nurses and other staff.
• Providing funding for the hospice volunteer program allowing families some needed free hours while trained volunteers provide care for their loved one.
• Raising awareness in the community about the benefits of hospice at end of life journeys.
